Creating, cloning and disabling forms

For admissions admins who set up application, supplementary and other forms.

I want to Go to
Create a new form Settings > Forms > + Create new form
Copy an existing form Settings > Forms > Clone Form
Stop families filling in a form Form editor > Enabled

Create a form

  1. Go to Settings > Forms and select + Create new form.

  2. In the Forms Editor, link the form to its Campus and Programme.

  3. Choose the form Type (see the table below).

  4. Set the layout and notification options:

    • Render form in 2 columns: shows fields in two columns on the PDF version of the form.
    • Parent email notification: lets parents choose whether to receive email notifications. If unticked, parents cannot change this and receive notifications by default.

  5. Drag fields from the menu on the left onto the form, and drag them to change their order. Double-click a field to edit it. See Adding and editing fields on forms.

Form type Use it for
Application The main application form
Supplementary Extra information you need, such as health or bus forms
Questionnaire Recommendation forms that parents send securely to recommenders
Tour Information you need before a tour. If this form is disabled, tour sign-ups are asked for basic information only.
Re-enrolment Collecting new information from parents, or confirming existing information, for returning students

Reuse the same field on every form that needs that information. Fields on more than one form share their data, so families fill them in once. For example, if Nationality is completed on one form, it is filled in on the next, and a change on one form updates all forms for that applicant or parent.

Global fields, such as First Name and Language, are part of the system structure. You can change only their label and instructions. Most other fields can be set as required, office-use only, given a default response or made conditional on another field.

Clone a form

Cloning is quicker than starting from scratch when you need a similar form.

  1. Go to Settings > Forms.

  2. Hover over the form and select Clone Form.

  3. Select the new copy to edit it.

Disable a form

Open the form and untick Enabled. The form is hidden from families, and the information already collected stays in OpenApply.
