2. The teacher receives an e-mail with a unique link, which takes them directly to the online form for that particular student
3. The teacher completes the form online. You receive the response electronically, and can view the data in OpenApply (via the 'More' tab in the top-right of the profile for that applicant)
4. The teacher receives an e-mail confirming they have submitted the form. The parent cannot see the responses of the teacher, as it is confidential
Select Questionnaire as the Type, and give your form a Title.
You can choose to select the option Allow parents to print as PDF, in case the teacher is unable to complete the form online, then the parent can export the recommendation as a PDF for the teacher to complete. Disable this option if you wish to ensure that the teacher completing the recommendation must complete the recommendation online.
The Applicant & Family page of the form shows the fields that the parents must complete, to send the recommendation request to the current teacher (or other educational colleague) that they are requesting the recommendation from.
When the parent completes this part of the form, they can choose to Export the recommendation form to PDF (if this option is allowed above), or to Save & Send via E-mail to the current teacher (or other educational colleague).
The Educational Colleague page of the form shows the fields that the current teacher (or other educational colleague) must complete. Fields can be edited in the Questionnaire Form grouped field, or new fields can be created by clicking + Add Field.
The current teacher will receive a unique link via e-mail to allow them to complete the form online, if the parent selects to Save & Send via E-mail. Once the teacher has completed the form, their responses can be viewed confidentially by the administrator, via the applicant profile. The teacher's responses are not visible to the parent.
It is necessary to add a Questionnaire type form to a Checklist, in order for a parent to be able to access and complete their part of the form.
